Patient Information - Your AdmissionYour doctor will have provided you with an admission pack that needs to be completed and returned to the hospital to confirm your booking. If you have not received this paperwork please contact Calvary John James Hospital, main reception on (02) 6281 8100 and we can either post this document to you or can take your details over the phone. Please have your medicare card and private hospital details at hand. If you are having difficulty in completing this form or have any queries please contact the main reception of the hospital on the number above. The admission pack provides us with the information to finalise your booking and to let us know about any special care you may require. It provides you with information about your hospital bill and how you can pay your account and general information about the hospital to assist you whilst you are with us. It also provides you with information on how we protect your privacy and information on your rights and responsibilities whilst in hospital. There are three forms in the admission pack:
